WORTHINGTON QUIT TOBACCO PROGRAM

Additionally, through our Medical Center, you can complete the six-week Worthington Quit Tobacco program, which uses the American Lung Association’s Freedom From Smoking Plus Curriculum with an optional 30-minute teleconference with Worthington’s Quit Specialist, Ann Burk. Completion of this program allows you to avoid the $40 Tobacco Surcharge. Eligible participants include employees, spouses, and dependent children between the ages of 18 and 26. Learn more here.

Once you complete the program, file a claim with Anthem to have up-front costs reimbursed.
