Worthington Enterprises offers two Delta Dental plan options: the Value Plan and Premium Plan. You may choose dentists in the Delta Dental Premier and PPO networks.

ID Cards
You’ll get two dental ID cards after you enroll. Each card will only show the employee’s name, even if your dependents are covered. You don’t need your ID card to receive services — just tell your dentist that you have Delta Dental PPO Plan.

This plan covers:

- Restorative procedures —fillings and extractions
Premium Plan
This plan covers:
- Preventive services — periodic exams and cleanings
- Restorative procedures — fillings and extractions
- Major procedures — root canals and crowns
- Orthodontia for children under age 17 is also covered.
